EBP Workbook - the Basics - Introduction

實證照護最好的定義就是：應用現今最好的臨床研究證據來照顧病患。

• The application of the best available evidence to patient care.
• EBM is the integration of best research evidence with clinical expertise and patient values . (Dave Sackett)

課程將分為初級及進階課程。

1. 初級課程：介紹實證五步驟及快速閱讀論文（利用 GATE frame 及 RAMbo；主要介紹治療研究）。
2. 進階課程：如何進行 Critical Appraisal（詳細介紹診斷、病因、預後、治療及系統性回顧等各類研究）並將結果寫成 CAT (Critically Appraised Topic) 並進行簡報或討論。

基礎課程目標：

1. 建構可回答的臨床問題。並分辨解決各類臨床問題，及最合適的臨床研究為何。
2. 瞭解如何選用資料庫解決各種臨床問題。
   Primary database (MEDLINE, CINAHL, Embase, The Cochrane Central Register of Controlled Trials (CENTRAL))
   Secondary database (Uptodate, MD Consult, Clinical Evidence, Dynamed, ACP PIER, Guideline)
3. 如何建構 Medline/PubMed 的檢索策略：
   textword (keyword) 及 MeSH (subject heading) 的異同及 automatic term mapping (ATM)
   Booleans (AND, OR, NOT) ； NEAR/ADJ 及 truncations (* and $)
   limit (clinical queries; & methodological filter, e.g. publication type, sub-type)
4. 如何有效率地評讀臨床研究。介紹 GATE frame 及 RAMbo（初級課程）；如何進行各類研究的 Critical Appraisal（進階課程）
5. 如何解釋或說明臨床研究的結果，並用於臨床決策。
   瞭解 RRR, ARR, NNT 及 NNH（初級課程）；瞭解 pre-/post-test probability, sensitivity, specificity, predictive value, likelihood ratio（進階課程）。
